§ 6-46. Same—Application.

(a)

Application for a permit to install, alter or change electrical equipment in the city shall be filed with the city inspector on printed forms furnished by the city clerk.

(b)

The application shall be accompanied by plans and specifications for the proposed work and shall include the following:

(1)

The name of the owner or lessee of the premises on which the work is to be done.

(2)

The location of such premises by correct street address or other information by which the premises may be readily located.

(3)

The purpose or purposes for which the permit is to be issued.

(4)

Such other information as may be required by the city inspector.
